Instead they may have signs of irritation, anemia, Anxiety, Depression, Skin Rashes, Ulcers, joint pain and bone related problems like Osteoporosis.   • Actually celiac disease damages the tiny hair like structure (villi) present in the small intestine making it unable to absorb the essential nutrients from the foodstuff. This would cause impairment of growth affecting the general health condition producing weakness due to mal-nutrition.
  • The chance of getting celiac disease is much higher if you have family history of this condition.
Who Are At Risk ?

People with Digestive Disorders and autoimmune disorders like Thyroid problems can develop this disease. The chance of getting this problem is more for children with Down’s syndrome and adults with Diabetes.

Sometimes, he may do a biopsy on the small intestine for removing small tissue to detect gluten intolerance. This is normally done by endoscopic method inserting a small tube into the mouth and sending it deep into the intestine for cutting tiny tissue.

Celiac disease cannot be treated but your doctor will tell you methods to manage the right nutritional level in your blood. Foods containing barley, wheat, rye, bulgur, semolina, oats, graham flour and spelt should be avoided. People affected with celiac disease should consult a dietician for eating balanced nutrition without the above products. The symptoms of abdominal pain and inflammation of small intestine can be managed by taking anti-inflammatory drugs or steroids. Inflammation of small intestine will gradually get reduced once the person stops taking gluten in daily food. However it can take several months for the symptoms to subside.

  • Avoid barley, wheat, and rye. Instead, go for quinoa, gluten-free oats, rice, millet, corn, buckwheat and breads made with potatoes or corns.

  • Fruits and vegetables are great choices. Vegetables such as zucchini, peas, green beans, lettuce, eggplant, onions, cauliflower, carrots, and peppers are rich in minerals and healthy for your kid. As for fruits, pay attention to variety for the balance of nutrition. Kiwi, watermelon, star fruit, papaya, mango, clementines, and pomergranate are some of the examples.

  • Most meats such as beef, pork, and chicken are gluten-free. But be careful with processing meat and read labels first.

  • Plain milk, yogurt, sour cream,cheese and cottage cheese are free to go. But do read labels when buying yogurts as some additives may contain gluten.


You may even have to consult with a dietitian to plan a better gluten-free diet for your child.Once you take gluten out of your child's diet, the intestinal inflammation will begin to lessen in a few weeks. However, it will take months, or years in some cases, for the villi to improve. If you accidentally give your child something that contains gluten, it will result in diarrhea and severe abdominal pain. Even trace amounts of gluten are enough to trigger an immune response.


Supplementation



In case your child shows signs of malnourishment, your doctor may suggest giving them vitamin and mineral supplements. You may have to give your child supplements containing iron, folate, calcium, vitamin K, zinc, vitamin D, and Vitamin B-12.


How Can I Help My Child?
Since your child is unaware of what's happening to him/her, it is important for you to take some steps and help your child understand his/her condition better. Here's what you can do for celiac disease in children.


1. Explain It to Your Child
You should explain it to your child why he/she needs to avoid gluten. Be sure to make it clear that they only need to avoid gluten and enjoy everything else. Talk to them regularly to help them develop the habit of avoiding gluten.


2. Help Them Develop Skills
It is important to make your child a part of the process, so they could develop skills to manage their condition better. If you always dictate their diet, they may push back and resist. Ensure their involvement in food shopping, meal planning, placing restaurant orders, reading labels, and packing school lunches. Empower them to make their own food, so they know what they can or cannot eat.


3. Help Them Read Labels
It is important to develop a habit of reading labels when you have a child with celiac disease. Help your child to do the same because sometimes gluten is present in food in different forms. Avoid anything like malt syrup and brewer's yeast. Gluten may also be present in salad dressings, sauces, vegetarian meat, and deli meats. Even oats can be contaminated when they are manufactured in a facility that also process gluten.


4. Discuss Everything with Teachers and Coaches
Talk to your child's school administrators and explain everything about their condition. Also, ensure that your child's coaches and teachers know about your child's diet for sports, classroom celebrations, and after-school activities.


5. Focus on Fun, Not Food
With so much to take care of, it is common for children with celiac disease to feel left out at birthday parties and school celebrations. It is your responsibility to make it fun and help your child understand that just because they cannot eat what other children are eating doesn't mean they're different.

Other diseases which can be confused with celiac disease are Irritable Bowel Syndrome or Inflammatory Bowel Disease, diverticulitis, infections of the intestine, chronic fatigue disease and Iron-deficiency Anemia which is caused by menstrual blood loss. This causes celiac disease to be wrongly diagnosed or under diagnosed. Various celiac disease tests can be prescribed when more symptoms are revealed.


1. Antibody Blood Tests
One of the important celiac disease tests is the antibody blood test which checks for antibodies specific to celiac disease. The most preferred test is IgA anti-tissue transglutaminase antibody. Other tests are to detect the presence of IgA endomysial. Another antibody – AGA is also tested; however, positive result does not necessarily mean presence of celiac disease. But it is good to determine the response to treatment. The level of AGA falls when you follow a gluten free diet.


Note: Don't go for gluten-free diet before the test. When one stops eating foods including gluten, the test results may be negative for celiac disease no matter one has the disease or not.


2. Intestinal Biopsy
If the blood test reports suggest celiac disease, biopsy is performed. A small piece of the small intestine is removed by the doctor and damage to the villi of the intestine is checked. The biopsy is performed by inserting an endoscope through the mouth and stomach till it reaches the small intestine. The endoscope has instrument by which a tiny piece can be removed to be checked for damage.


3. Dermatitis Herpetiformis
Around 15 to 20% of people suffering from celiac disease have dermatitis herpetiformis. It is an intensely itchy rash which causes skin blisters and occurs on elbows, knees and buttocks. People with this symptom usually do not display any digestive symptoms associated with celiac disease. This is also diagnosed by performing blood test and skin biopsy. Anti-body tests and skin biopsy can usually determine DH. Antibiotics are prescribed along with gluten free diet.


4. Genetic Testing
One or both of HLA DQ2 and DQ8 genes are usually seen in people with celiac disease. But it is also present in 40% of normal individuals, hence the presence of these genes does not mean a person has or will develop celiac disease. It is done when other tests are inconclusive and/or are relatives. The test can be done by blood tests, testing saliva or taking cheek swabs.


Celiac Disease Treatment
Once the celiac disease test is performed and the diagnosis is confirmed, one of following methods of treatment can be used:



  • Gluten-free diet: Once gluten is eliminated from the diet, the symptoms gradually disappear and should be maintained lifelong. The villi of the small intestine will need some time to get healed, for adults that time would usually be 2 years. It is important to test for nutritional deficiencies for iron, folic acid, vitamin B12 & D.

  • Consult your dietitian: A dietitian can help in identifying foods which contain gluten and also help in planning meals without gluten. You can make proper choice when grocery shopping etc.

  • For unresponsive celiac disease: When people do not respond positively to gluten free diet, it is referred to as unresponsive celiac disease. The damage to the intestine is severe and there are chances that it might not heal. Intravenous nutrition might be required for such individuals and gastroenterologists might be consulted.
